September saw a devastating fire at a garment factory in Pakistan that left at least 289 people dead. Officials billed the inferno, which bore grim echos of the 1911 Triangle shirtwaist factory fire in New York City, as the deadliest industrial accident in the country’s 65-year history. Employees who were on-site when the fire broke out had few options of escape: every exit save one was locked. Most of the windows were also barred, creating what officials later described as a “death trap.”
